Entry Fee only $15.00 per person ($10.00 for children 12 and younger)

Please make checks payable to Kingston Sunrise Rotary and mail with the registration form below to P. O. Box 4262  Kingston, NY 12402

Your non-refundable entry fee includes:

            18 holes of miniature golf

            Gifts

            1 Raffle ticket for prizes (additional raffle tickets available for purchase at

the event)

            BBQ Dinner (starting at 6:00)

            Sports Towel

2 Trophies will be awarded -   One for the winning team of each round of golf.
